Certus Healthcare operates seven Skilled Nursing & Rehabilitation communities providing services from Skilled Nursing Care, Rehabilitation, Long Term Care and Alzheimer’s
Dementia Care to Behavioral Health.
Certus Healthcare works with multiple home- and community-based services to transition patients from rehabilitation back to the community. We work hard to find programs and services that meet the needs of our patients who wish to return home safely and comfortably.
